RCO is designed to help connect the community to resources by promoting community tenure that may be holistically beneficial and supporting overall personal wellness. RCO offers planned linkage and advocacy in partnership with several community providers to offer professional support for the member.

Our collaboration of services and resources are to provide a continuum of care, individualized for each participant. RCO works with the participant and the provider, in accordance with a plan of action that is developed and approved by the participant and RCO, in order to reach their individual needs. By providing personalized care for the participant, we make it possible for the participant to remain successful in the community and in their daily living.
